A PDF of your transcript to upload (form will not accept pictures, must be PDF)
Confirmation Number for MPP Payment (for New Member applicants only)
*Recommend completing on a CPU, not a phone
Applications and payment MUST be submitted by end of day September 2nd. Any applicants who do not meet entry requirements will be receive a refund of the dues less a $5 processing fee.
